Race Forward takes seriously its obligation to safeguard the health of its employees, contractors, clients, and the community at large from COVID-19. As we navigate the next phase of the pandemic and contemplate our organization’s return to in-person gatherings, we endeavor to ensure we foster a community of care.

Please use this portal, by guidance received from Race Forward, to upload

• proof that you are up to date on your COVID-19 vaccinations; OR

• proof of a negative test for COVID-19 taken within the 72 hours immediately preceding your in-person participation at a Race Forward event.

“Up to date” means a person has received all recommended doses in their primary series of COVID-19 vaccines at least 14 days before the scheduled date of the Event and a booster dose when eligible. Per Centers for Disease Control (CDC) guidance, boosters are recommended five months after completing your primary COVID-19 vaccination series if you received the Pfizer-BioNTech or Moderna vaccines and two months after receiving the single-dose Johnson & Johnson vaccine.

The required COVID test must be an antigen or molecular test (rapid, nucleic acid, RNA, or PCR). A laboratory, healthcare, or test provider can provide proof of a negative test. Home tests are also acceptable.

All medical information collected from individuals, including vaccination information and test results, will be treated by applicable laws relating to confidentiality and privacy.

We recognize the COVID-19 landscape is changing rapidly and will continue to monitor the evolving situation and update health and safety guidelines accordingly.
