July 2023 Vaccination Proof for NorthStar Education Partners at Promise 54 Retreat

In alignment with our COVID travel policies, please review the following information:

- All staff and contractors must be fully up to date on their COVID-19 vaccinations, defined at this point and based on CDC guidance as:

• 2 weeks after 1 dose of the single-dose J&J vaccine with booster 6 months later and 2 boosters for those age 50 and over.
• 2 weeks after 2nd dose of the Moderna or Pfizer vaccine with booster 6 months later and 2 boosters for those age 50 and over.

- Proof of vaccination should be uploaded into CrowdPass prior to the travel event.

- Please use a photo of your CDC card or another form of official documentation (pharmacy, doctor’s office, state vaccination record) that includes the dates of your initial dose(s) and your booster(s)

- Individual members may request to opt out of the requirements above for medical reasons or religious beliefs. Accommodations must be approved in writing by the CEO. Individuals with approved accommodations will be subject to additional restrictions and requirements to ensure the health and safety of the Promise54 community, which may include, but is not limited to, COVID-19 testing or not attending in-person activities.
