To upload your COVID-19 vaccination record as required by our Vaccination Policy for this event, please click the blue "Register" button to the right.

As a reminder, we are requiring individuals to be up to date with all vaccinations against COVID-19 in order to gain entry to the conference. This means they have received all recommended doses in the primary series *and one booster* when eligible. Getting a second booster is not currently required.


***MEDICAL OR RELIGIOUS EXEMPTION NEEDED?***
If you require a medical or religious exemption from this vaccination requirement (from either the booster or vaccination in full), do NOT complete your registration via the link below. Instead, email your exemption letter to: ashley@crowdpass.co, with the subject line: "ATTENTION ASHLEY - Innovations and Solutions for Ending Unsheltered Homelessness". Please ensure your email contains your First & Last Name as it appears on your conference registration and your exemption letter for approval.

Before sending your email, please review the letter requirements below. Please allow up to one working week to receive approval for your exemption documentation. As a reminder, you will also be required to submit a negative COVID-19 PCR or antigen test from a reputable clinic 48 hours before the conference.

ALL EXEMPTION REQUESTS MUST BE MADE NO LATER THAN FEBRUARY 15, 2023.

**Approved Medical Circumstance:** History of previous allergic reaction to indicate an immediate hypersensitivity reaction to a component of the vaccine; health condition or medical circumstances such that immunization is not considered safe; or any other health condition or medical circumstance where receiving the COVID-19 vaccine is contraindicated

-Document Requirement for Medical Circumstance: A letter from your medical provider explaining the medical reasoning prohibiting your receipt of the COVID-19 immunization


**Approved Sincerely-Held Religious Belief:** Religious belief and/or practice which is (a) sincerely held; and (b) which prohibits receiving immunizations.

-Document Requirement for Religious Belief: A letter from your religious leader (priest, preacher, rabbi, imam, etc.) explaining the religious teachings prohibiting receipt of the COVID-19 immunization
