About Event
We are so excited to host you at the athenahealth 2023 Executive Circle in Phoenix, Arizona. For all external guests attending, we have partnered with CrowdPass to verify your vaccination or COVID-19 test details. To get started, please review all the guidelines listed below. Once you are ready to upload your documents, please select the “Register” button on the right-hand side.
Please note that athenahealth employees do NOT need to complete this – it is for external guests only.
In accordance with athenahealth's Conference and Event Code of Conduct, all external guests must present prior to entry either: (1) proof of COVID-19 vaccination, or (2) a negative COVID-19 test result administered within 48 hours prior to the event, accompanied in either case by a valid form of government-issued identification. Acceptable forms of COVID-19 tests include a negative antigen or nucleic acid amplification (including PCR) test administered through a local pharmacy, medical provider, or other licensed testing provider. At-home test results will not be accepted.
Individuals are considered “fully vaccinated” two weeks after completing the second dose in a two-dose series, such as the Pfizer or Moderna vaccines, or two weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine. A booster dose of the vaccine is not currently required to be considered fully vaccinated.
If you are fully vaccinated, you may submit your proof of vaccination at any time prior to the event. If you are submitting a negative covid test, you can upload your results 48 hours prior to the start of the event, regardless of what date you are planning to arrive in Arizona. The event begins on Thursday, March 2nd so you may begin uploading your test results starting anytime on Tuesday, February 28th.
